Welcome to Trinity Hill Care Center, a skilled nursing and rehabilitation center located in Hartford, Connecticut. Trinity Hill provides short-term rehabilitation, skilled nursing, long-term care and specialty services for individuals with a wide range of medical, behavioral and social support needs.

Located in Hartford’s South End, Trinity Hill serves residents from throughout Greater Hartford and communities across Connecticut. Our interdisciplinary team works to provide individualized care and support with a focus on each resident’s clinical needs, personal goals, independence and quality of life.

Specialized Care for Complex Needs

Trinity Hill has developed experience caring for individuals whose needs may extend beyond those typically addressed in a traditional nursing center. Our clinical and support teams work together to care for residents with complex medical conditions as well as behavioral health needs, substance use disorders and HIV, including individuals experiencing a combination of these needs.

We also collaborate with hospitals, physicians, community providers and other healthcare and social service organizations to help coordinate care and support successful transitions.
