Under the supervision of Trinity Hill’s Medical Director and Director of Nursing, our nursing team provides individualized care and monitoring for residents with a wide range of medical conditions and clinical needs.

Post-Hospital & Post-Surgical Care
The transition from a hospital to the next level of care is an important part of recovery. Trinity Hill provides skilled nursing and sub-acute services for individuals who are medically stable enough to leave the hospital but continue to require nursing care, monitoring, rehabilitation or other clinical services before returning home or transitioning to another setting.
Care plans may incorporate nursing services, medication management, wound care, IV therapy, rehabilitation and education based on each resident’s needs.
For residents who require therapy following hospitalization, Trinity Hill also provides physical, occupational and speech therapy through its comprehensive rehabilitation program.
Complex Medical Care
Some individuals require a skilled nursing center capable of managing multiple medical diagnoses and clinical needs simultaneously.
Trinity Hill’s interdisciplinary approach allows nursing, rehabilitation, medical providers, dietary services, social services and specialty programs to work together around a coordinated plan of care.
This approach can be particularly important for residents managing chronic or complex conditions that require ongoing clinical monitoring while also working toward improved function, greater independence or a successful transition to the community.
Cardiac, Pulmonary & Renal Care
Trinity Hill provides nursing care and rehabilitation support for residents recovering from or living with cardiac, pulmonary and renal conditions.
Individualized plans of care may incorporate medication management, clinical monitoring, rehabilitation, dietary support, education and coordination with outside specialists and dialysis providers when appropriate.
The goal is to support each resident’s medical stability while maximizing function and quality of life.
Wound Care & IV Therapy
Residents requiring advanced nursing interventions can receive wound care and IV therapy as part of their individualized plan of care.
Trinity Hill’s nursing capabilities include skin and wound assessment and management as well as IV therapy utilizing PICC and central lines. Nursing staff coordinate with physicians and other clinical providers to monitor treatment and respond to changes in condition.
Specialty Care for Complex Needs
Trinity Hill also offers specialized programs for individuals whose needs extend beyond traditional post-acute nursing care.
Our dedicated HIV Specialty Care Program provides skilled nursing and long-term care supported by infectious disease expertise, while our Behavioral Health Program combines nursing care with psychiatric, behavioral health and psychosocial supports.
These specialty capabilities allow Trinity Hill to consider referrals involving combinations of medical, behavioral health and social needs that may require a more comprehensive approach to skilled nursing care.
