Trinity Hill Care Center provides comprehensive rehabilitation services for individuals recovering from illness, injury, surgery or hospitalization. Located in Hartford, Connecticut, our rehabilitation program combines skilled nursing care with physical, occupational and speech therapy to help each resident regain strength, mobility, function and independence.

Rehabilitation services are available seven days a week through our Touchpoints Therapy team, allowing residents to receive consistent support throughout their recovery.

Physical, Occupational & Speech Therapy

Every rehabilitation journey is different. Trinity Hill’s therapists develop individualized treatment plans based on each resident’s abilities, clinical needs and goals.

Our rehabilitation services include:

Therapy takes place in Trinity Hill’s fully equipped rehabilitation department and is coordinated with nursing and other members of the interdisciplinary care team.

Physical Therapy

Physical therapy focuses on helping residents improve strength, balance, endurance and mobility following a hospitalization, illness, injury or procedure.

Depending on individual needs, therapy may focus on walking and transfer training, balance, strengthening, endurance and the safe use of assistive devices. The goal is to maximize each resident’s functional ability and help prepare them for the next stage of recovery.

Occupational Therapy

Occupational therapy helps residents regain the skills necessary to perform everyday activities as safely and independently as possible.

Therapy may address dressing, bathing, grooming, mobility and other activities of daily living. Occupational therapists can also provide education on adaptive equipment and techniques that may make everyday tasks safer and more manageable.

Speech & Language Therapy

Speech and language therapy can assist residents experiencing communication, cognitive or swallowing difficulties.

Treatment is individualized according to each resident’s needs and may address speech and communication, cognition, memory, problem-solving and swallowing function.

Preparing for a Successful Return Home

For many short-term rehabilitation residents, the ultimate goal is a safe return home.

Discharge preparation begins during the rehabilitation stay and brings together therapy, nursing and social services. Residents and families receive education and support designed to help prepare for the transition from Trinity Hill to home or another appropriate setting.

Recommendations may include adaptive equipment, continued therapy, community-based services or other supports based on the resident’s individual needs.

Rehabilitation with Access to Specialized Care

Trinity Hill’s rehabilitation program is supported by the clinical capabilities of a skilled nursing center. This can be especially valuable for individuals whose rehabilitation needs are accompanied by complex medical or behavioral health conditions.

In addition to rehabilitation, Trinity Hill offers skilled nursing and sub-acute care as well as specialized behavioral health and HIV care. This interdisciplinary environment allows rehabilitation goals to be coordinated with the resident’s broader medical and psychosocial needs.

Rehabilitation Referrals & Admissions

Trinity Hill accepts rehabilitation referrals from hospitals, physicians, healthcare providers and other referral sources throughout Greater Hartford and Connecticut.
