Trinity Hill Care Center provides specialized skilled nursing and long-term care for individuals living with HIV. Located in Hartford, Connecticut, our dedicated 30-bed HIV Specialty Care Unit combines skilled nursing services with infectious disease expertise, individualized treatment, behavioral health support and comprehensive social services.

Trinity Hill is one of only two dedicated HIV units recognized by Connecticut regulatory authorities as a specialized long-term care HIV program. Our interdisciplinary approach is designed to address not only each resident’s HIV diagnosis, but the medical, behavioral health, recovery, nutritional and social needs that can influence long-term health and quality of life.

Specialized HIV Medical and Nursing Care

Trinity Hill’s HIV program operates under the direction of an Infectious Disease physician specialist who conducts clinical rounds at the center weekly and is available 24/7 for consultation regarding program residents.

Care and clinical services include:

• Individualized HIV medication and antiviral regimens

• Skilled nursing care provided by consistent unit staff to support continuity of care

• On-site APRN services

• Infectious Disease physician consultation and oversight

• IV therapy

• Complex wound care

• Physical, occupational and speech therapy

• Nutritional counseling with an on-site dietitian

Each resident’s plan of care is individualized according to their medical needs, functional abilities and personal goals.

Behavioral Health, Recovery & Emotional Support

Living successfully with HIV can involve needs that extend well beyond medical treatment. Trinity Hill integrates behavioral health, counseling, education and recovery support into its specialty program.

Residents have access to services that include:

• Individual mental health counseling

• HIV education and support groups

• Grief, coping and recovery-skills programming

• Individual and group substance use counseling through the iRecovery program

• Social services and individualized discharge planning

This integrated model allows the Trinity Hill team to address co-occurring medical, behavioral health and substance use needs as part of a coordinated plan of care.

Social Services & Community Transition Support

For residents whose goal is to transition back into the community, successful discharge can depend on access to housing, healthcare, benefits and ongoing social supports.

Trinity Hill’s dedicated HIV Program Social Worker assists residents with discharge planning and connections to community resources. The program also coordinates with community organizations and support programs, including resources available through Ryan White funding and Money Follows the Person.

Transitional counseling, post-discharge follow-up and social services support are incorporated into the program to help residents prepare for the next stage of care.

Connected to Connecticut’s HIV & Community Care Network

Trinity Hill works with healthcare providers, community organizations, housing resources, behavioral health providers and substance use treatment organizations throughout Connecticut.

These relationships help the care team coordinate the medical and social services that residents may need during their stay and following discharge.

Referrals & Admissions

Trinity Hill’s HIV Specialty Care Unit accepts referrals from hospitals, healthcare providers, community organizations and other referral partners for individuals requiring skilled nursing or long-term care.
