A Letter From The Administrator
Dear Resident and Family members,
It is my great pleasure to welcome you to Trinity Hill Care Center. Our home has been an integral part of the Greater Hartford Community for well over thirty five years and we remain committed to strive to meet and exceed your expectations. Additionally, we understand the importance of partnering with you to ensure the care and services you receive are clearly defined according to your individual plan of care.
We provide a variety of programs specially designed to meet your needs and goals. Our Rehabilitation Program is designed to meet the needs of those who have experienced a physical or cognative loss of function. Your care givers will assist you in restoring maximum independence geared towards resuming a satisfying, productive life. Our Behavioral Health Program assists those individuals experiencing emotional challenges and social difficulties. The program is coordinated by the Director of Resident Psycho-Social Services and a team of highly qualified individuals who provide short and long term psycho-social services within a secure and caring environment. The HIV/Aids Program at Trinity Hill Care Center is designed to provide optimum quality of life to individuals with HIV, AIDS and related substance abuse challenges. Medical Care is coordinated by a physician who specializes in Infectious Disease and includes IV Therapy and Pain Management. In addition we also provide post-acute medical services, wound care, Hospice and Respite services.
Trinity Hill Care Center is the proud recipient of the Quality First Award (October 2008) from the American Health Care Association (AHCA). This award is clearly indicative of our strong commitment to quality care and continuous quality improvement. I am proud of our team of highly skilled and dedicated employees.
